Online: Submit a claim form at the official settlement website, requiring the Notice ID from the settlement notice and details such as email addresses associated with the FuboTV account, approximate account creation and closure dates (if applicable), and the years the claimant watched FuboTV content.
  2. Mail: Download, print, and complete the PDF claim form from the settlement website and mail it to:
